Background Check Authorization

1. Authorization

I authorize Waly Market and its authorized background-screening provider to obtain consumer reports or investigative consumer reports about me for legally permitted purposes related to my eligibility to provide delivery services, subject to applicable law.

2. Information

I understand that I may be asked to provide identifying information necessary to conduct the screening and that the information will be handled according to applicable privacy and consumer-reporting requirements.

3. Certification

By signing or electronically accepting this authorization, I certify that the information I provide for the screening is accurate to the best of my knowledge.

4. Applicable rights

This authorization does not waive any rights provided by federal, New York State, New York City, or other applicable law.
